Fractionated coconut oil (FCO) is a fraction (portion) of the whole oil. Also referred to as caprylic/capric triglyceride oil or medium-chain triglyceride (MCT) oil because it is primarily the medium-chain caprylic (8 carbons) and capric (10 carbons) acids that make up the bulk of the oil. The subject was a 6-year-old white female with a history of eczema since birth with recurrent flare-ups. The rash was itching and spreading. Vaseline did not relieve her dry skin. Mother requested “a more natural alternative to steroids."
On exam: She had multiple white-pink dry, scaly excoriated plaques generalized on her trunk, neck, face, and extremities. PGA was severe and VAS was 9/10.
Plan: Severe atopic dermatitis. She was placed on CapriClear BID. Follow-up exam 4 weeks later showed decreased redness & scaling and greatly improved skin. Several post-inflammatory hypopigmented patches remained. Her itching had resolved. PGA was excellent and VAS was decreased to 1/10.
